Property separation agreement
A property separation agreement causes each spouse to maintain full property independence. Each manages their personal property, and no joint marital property is formed. Liability for one spouse's debts only applies to their property.

- Marriage certificate extract
- Personal data of spouses (first names, last name, parents' names, date of birth, PESEL number, residential address, ID card or passport number and expiry date)
